If you have a multiple staff, make as many copies of this form as needed and please indicate Sports Editor. The membership dues are $10.00 per person (members of the print media or retired from the print media).

Some areas of the state are getting a new area code after February 1, 2008. Please give the new area code with your phone numbers.
